All on 4 Revision with Zirconia Bridge: When a Smile Finally Feels Right

After years of living with an All on 4 result that never felt right, one patient found renewed confidence through a zirconia bridge revision at ArtSmiles.

When Your All on 4 Does Not Feel Like You

Getting dental implants is a big decision. So when the results do not match your expectations, it can feel deeply disappointing. That was the reality for one of our patients, a woman in her mid-sixties who had received All on 4 treatment at another clinic roughly seven years earlier.

Her implants were healthy and well-placed. But the bridge sitting on top of them told a different story. The teeth projected too far forward, giving her profile an unnatural look. The shade did not match her complexion. And her bite never quite felt comfortable.

Over time, these issues took a toll. She stopped smiling in photos. She avoided social situations. Something that was supposed to give her confidence had quietly taken it away.

A Fresh Set of Eyes on Existing Implants

When she came to ArtSmiles, our team started by listening. We wanted to understand not just the clinical picture but how she felt about her smile and what she hoped to change.

After a thorough assessment, including CBCT scans to evaluate the position of her existing implants, we had good news. The implants themselves were stable and well-integrated with the bone. There was no need to remove or replace them.

The problem was entirely with the prosthetic design. The bridge needed to be reimagined from scratch.

Redesigning the Bridge with Precision

Our approach focused on three key goals:

  • Correcting the projection so the teeth sat in a natural position relative to her lips and facial profile

  • Choosing the right shade to complement her skin tone and look authentic

  • Improving the bite so her upper and lower teeth met evenly and comfortably

We designed a new monolithic zirconia bridge. Zirconia is a high-strength ceramic material that closely mimics the look of natural teeth while being incredibly durable. Monolithic means the bridge is milled from a single block, which reduces the risk of chipping over time.

To work with the existing implant positions, we used angled screw channels. This allowed us to secure the new bridge at the ideal angle without compromising the final appearance. Every screw access hole was carefully planned so it would be hidden from view.

The Try-In Phase: Getting It Right Before Making It Permanent

Before finalising the bridge, we completed a try-in phase. This step let us place a prototype version in the mouth so we could assess the fit, the bite, and the overall look in real time. Our patient had the chance to see her new smile and share her thoughts before anything was made permanent.

This collaborative process made all the difference. She felt involved, heard, and in control of the outcome.

The Moment Everything Changed

On the day we placed the final bridge, something shifted. She looked in the mirror and smiled. Not a polite, closed-lip smile. A real one.

In the weeks that followed, she told us she had started wearing makeup again. She was going out more. She felt like people were seeing her again, not just her teeth.

That emotional shift is something we never take for granted. A smile is personal. When it feels wrong, it affects everything from your confidence to how you carry yourself in the world.

What to Know About All on 4 Revisions

If you have had All on 4 treatment and something does not feel right, it is worth knowing that a revision is often possible without starting from scratch. Here are a few things to keep in mind:

  • Healthy implants can often stay. If the implants are stable and well-positioned, the fix may only involve replacing the bridge.

  • Modern materials make a difference. Zirconia bridges offer a natural appearance and long-term strength that older acrylic options may not provide.

  • Precision planning matters. CBCT imaging and digital design help ensure the new restoration fits your face, not just your jaw.

  • Your input is part of the process. Try-in appointments give you a chance to see and approve the result before it is finalised.
